DECLARATION: I, the parent or guardian of the student named on this application, give permission for my child to leave the madrasah premises to participate in local visits and activities.

DECLARATION: In the event of an emergency requiring medical intervention, I, the parent or guardian of the student named on this application,  give permission for my child to receive First Aid treatment by a trained member of LII/LIC staff.

DECLARATION: I, the parent or guardian of the student named on this application, give permission for my child to travel to and from the madrasah alone.
